Our Care Coordinators help people with special treatment needs related to mental illness, substance abuse, or intellectual/developmental disabilities. Care Coordinators can:
- Help those at a high risk of being institutionalized, hospitalized or otherwise placed outside their home
- Ensure you receive the correct health assessments, treatment planning and support services to best meet your individual needs
- Work directly with you, your service providers and your community partners to keep you healthy and thriving. This includes ensuring you receive treatment from appropriate clinical and medical specialists as needed.
At Vaya, Care Coordination includes education and support opportunities though the Geriatric and Adult Mental Health Specialty Team. The team serves professional staff, family caregivers and other individuals who care for or work with older adults experiencing dementia or other behavioral health concerns.
