Primary Care Provider – Choice Period

The Primary Care Provider (PCP) Choice Period was April 15 – May 15. For assistance in changing a PCP, you may speak with your assigned Vaya care manager, call Vaya’s Member and Recipient Service Line at 1-800-962-9003 (7 a.m.t o 6 p.m., Monday – Saturday, including holidays), or complete the PCP Change Request Form.
